1.  For bookings, Allianz Healthbox members must contact CMI through email at [email protected] with the following information and documents:

• The promo code cannot be used in conjunction with any other discounts and promotions. In the purchase of goods and services which are on promotional discount, in compliance to Republic Act No. 9994 also known as “Expanded Senior Citizen Act of 2010” and Republic Act 10754 also known as “Act Expanding Benefits and Privileges of Persons with Disability”, the senior citizen or PWD can avail of the promotional discount, or the discount provided under the said laws, whichever is higher and more favorable.
• The promo code is for one-time use only and non-transferrable.
• Some tests, procedures, and treatments cannot be performed without doctor’s orders. In the case that doctor’s orders are not available, Allianz Healthbox members can avail consultation prior to any tests or procedures. The consultation fees are not subject to Allianz Healthbox members discounts.
• Cancellation of Dental bookings should not be later than 24 hours before the schedule. Same day cancellation or no-show fees will be charged to Allianz Healthbox member.
• Allianz Healthbox members may settle through the use of major credit cards, cash, Gcash and/or online bank transfer.
